1. Who can apply for these programs?: No one can be denied the opportunity to apply for these programs, however, there are eligibility criteria that must be met. Visit the Access Wisconsin website to see the specific guidelines for applying for each program.

2. What are the eligibility requirements for these programs?: The eligibility requirements are slightly different for each program. Eligibility is determined by family size, income, assets, and other household information. "Am I Eligible" is a tool located on Access Wisconsin Website that allows information to be entered to determine what programs a household may be eligible for.

4. Where and how do I apply?: Applications are available at Portage County Health and Human Services at 817 Whiting Avenue and can also be submitted online at Access Wisconsin Website. To have an application mailed or for any other questions regarding the application process contact the IM Central Consortium call center at 888-445-1621.
