Short Term Care/Subacute Care Service
Following hospitalization, the Portage County Health Care Center team provides medical and therapy services, bridging the transition between hospital and home. Services provided include: •Complex medical management

- Physical/ Occupational/ Speech therapies
Long Term Care
The Portage County Health Care Center cares for clients in need of a high degree of ongoing medical attention and/or supervision. Care staff work toward helping each individual achieve and maintain an optimal level of self-care and independence.
Dementia Care Advanced Stages
The Portage County Health Care Center offers specialized care for residents with Alzheimer's disease or other related dementia, who may require a high level of medical management and physical care. Nursing and Activity Staff receive additional training and provide attention to meeting physical, emotional and recreational needs of residents with advanced stages of dementia. Care is focused on offering support, reassurance and security to these individuals. Individualized activity programs are offered on the unit in smaller groups to stimulate and encourage mental activity.
Hospice Services
Hospice services are available to residents who need end of life care. Portage County Health Care Center works in partnership with Ministry Home Care Hospice and Aspirus Comfort Care & Hospice Services to provide this supportive end of life care. Hospice care is a covered benefit under Medicare, Medicaid, and some insurance plans.
Nursing Services
Registered Nurses, Licensed Practical Nurses, and Certified Nursing Assistants, under the direction of the resident's Physician provide twenty-four hour nursing service. Care plans are developed with the resident to meet the specific individual's needs and are designed to keep the resident functioning at his/her highest level, maximizing strengths and abilities and promoting the optimum level of wellness. Nursing is involved in the coordination of the services of the total health care team including the resident and his/her family in the planning and implementing of the nursing care program. Staff is provided with ongoing education and training to meet the special needs of each resident. This is accomplished through in-service training programs, developing employee skills and monthly meetings.
Physician Services
Each resident may be seen by their Physician of choice. If the Physician of choice does not make on-site visits, the resident can choose off-site visits. The resident can also be seen by the Attending Physician and/or Nurse Practitioner. The Attending Physician and Nurse Practitioner make regular visits and are available to meet with residents and families upon request by appointment in addition to routine scheduled visits.
Social Services
To find out more about admission to our nursing home, whether long or short-term, the Social Services staff is available to assist.
Social Services in cooperation with physicians, nursing personnel and other staff help to provide a full range of services. Social Service Staff will assist you in becoming familiar with and using the facility's staff and services. To ensure that social needs of residents and their families are fulfilled while at the Health Care Center, the Social Service Staff is available for admission and discharge planning, liaison with community resources and counseling with individual residents and family members. Social Services staff will make referrals to appropriate agencies whenever there are indications of financial, psychiatric, restorative, or social service needs or at the request of the resident or family member.
Rehabilitation Services
Rehabilitative Services are provided to all residents as ordered by the attending physician. Portage County Health Care Center contracts with Powerback Therapy for therapy services. These services include physical therapy, occupational therapy, and speech therapy.
Physical Therapy
The goal of physical therapy is to restore normal movement and function while attempting to resolve problems such as pain, weakness, loss of motion or lack of coordination. The benefits of physical therapy in a skilled nursing facility are restoration of mobility, maximizing function, improved safety awareness and assists in reaching the greatest level of independence in order to return to the community as soon as possible.
Powerback Therapy therapists work with the resident and families to ensure the plan of care developed is individualized and designed to offer the resident the best opportunity for a successful rehabilitation.

Occupational Therapy
Occupational therapists help individuals become as independent and productive as possible in performing the tasks that make up their daily lives. Powerback occupational therapy program maximizes the patient's ability to participate in the self-care, work-related, leisure and social activities of daily living.

Speech Therapy
Speech-Language Pathology seeks to restore or improve residents' communication abilities, speech/language deficits, hearing impairments and/or swallowing abilities. Many residents who have suffered from strokes, brain injury, Alzheimer's, or other neurological disorders often lose the ability to speak and may have difficulty understanding spoken and written material.
- Assists with management of swallowing and feeding difficulties
- Caregiver training to maximize communication, decrease confusion and/or promote safe swallowing
- Cognitive intervention to assist with memory deficits, judgment/reasoning difficulties and orientation deficits
- Use of compensatory strategies to facilitate speaking clearly, understanding others and improving word-finding difficulties
Out-Patient Therapy
Outpatient therapy is available for Physical Therapy, Occupational Therapy, or Speech Therapy with a physician's referral. We work closely with your physician to help you achieve your maximum independence.
Dietary Services
The Dietary Department is under the direction of a Certified Food Service Director and a Registered Dietitian. The experienced Dietary Staff prepares three nutritious, well-balanced, home-cooked meals daily. The Dietary Staff is eager to please each resident and will provide alternatives for food allergies/food preferences. Meals are served in the resident dining areas or in their rooms to meet individual needs. Appropriate snacks between meals are available throughout the day. Special theme and holiday meals are planned throughout the year. Residents' families are welcome to dine with them; guest meal tickets are available from the Business Office.
The Registered Dietitian assesses the nutritional requirements, develops and implements nutritional care plans to meet the individual needs of each resident. The Registered Dietitian also promotes optimal nutritional status and well-being of the residents through medical nutritional therapy and in accordance with the Physician's orders and consistent with their individual physical, ethnic, religious, and social needs. Therapeutic diets, written and maintained by the Dietitian, are served as prescribed by the Physician.

Laundry Services
Laundry Services include the washing of personal clothing and are provided without extra charge. We request that all personal clothing be properly labeled by marking clothing with a permanent marker or nametag. If you choose, your family can launder your personal clothing.

Religious services and programs are held regularly.
Beauty and Barber Services
A licensed beautician is available upon request. Appointments can be made through the beautician at 715-347-7330. A listing of prices is available through the Business Office.
