Statewide Issuance of Birth/Marriage/Death/Divorce Certificates
With the new statewide issuance law now in effect, if the vital record you are requesting falls within the dates listed below you can obtain the copies from The Portage County Register of Deeds office despite where the event took place in the state of Wisconsin.

Note: It is a felony to copy or reproduce these certificates per Wisconsin State Statute 69.24(1).
Expedited Service
For your convenience, you can process online requests through an independent company that the Portage County Register of Deeds has partnered with to provide you with this service; VitalChek. An additional fee is charged by VitalChek for this service, and all major credit cards are accepted, including American Express®, Discover®, MasterCard® or Visa®.
In-Person
Download and print one of the following Vital Record application forms.
- Complete all parts of the appropriate form, OR forms are available in our office for completion.
- Visit the Portage County Register of Deeds Office Monday through Friday, from 7:30 am to 4:15 pm
- Bring a valid photo i.d. (driver's license or government-issued i.d.)
- Acceptable forms of payment: cashier's check, money order, or cash.
Credit/Debit cards and personal checks are not accepted.
Regular Service via Mail
For birth or marriage certificates throughout Wisconsin and deaths, divorces or domestic partnership declarations/terminations that occurred in Portage County:
- Download and print one of the following Vital Record application forms.
- Complete all parts of the appropriate form.
- Enclose the following in an envelope:
- The completed form you printed.
- A self-addressed stamped envelope.
- Appropriate payment.
- A photocopy of your valid photo ID.
- Acceptable forms of payment: cashier's check, money order, or cash. Credit/Debit cards and Personal Checks are not accepted
- Mail the completed form and payment to:

Required Identification
Identification is required to pick up certified copies of vital records in person. A photocopy of the applicant's i.d. is required with mailed applications.
The acceptable forms of identification are:
One of These
- Valid State Issued driver's license or ID card
- Tribal or Military ID card
- Valid Government-issued employee ID card or badge with a photograph
- Passport
Or Two of These
- Bank/Earnings statement
- Current, dated, signed lease
- Current Utility Bill with accurate address
- Traffic ticket
- Hunting/Fishing license
- Vehicle registration/title
All forms of identification must show your current name and address. Expired cards or documents will not be accepted.
